Registered CertReady Health users may request deletion of their CertReady Health account and associated personal data. Submit the form below and our team will process your request.

Thank you. Your account deletion request has been received. We may contact you at the email address you provided to verify that you own the account before your account and applicable data are deleted or de-identified. If you have an active subscription, remember to cancel it through your app store (see below).
What happens after I submit a request?
- CertReady Health may take steps to verify that you own the account associated with the request before processing it.
- Your account and the applicable personal and study data will be deleted or de-identified as appropriate.
- Certain limited information may be retained where required or permitted for legal, accounting, fraud-prevention, dispute-resolution, security, or similar legitimate purposes.
